How can I get around medical insurance red tape?

I was supposed to have a vein closure, but the insurance company apparently messed up my paper work. I’m trying my darnest to get this done before I start my new job. The surgery is rescheduled for a couple says, but the insurance company keeps delaying. I know that varicose veins can be considered cosmetic, so says the insurance and a lot of different web sites, but in my case it is more, being that I am in pain on a daily basis with this ridiculous situation.
